As a female, if you are facing thinning and losing hair, you are probably pretty stressed by it. However, you will not be worrying and causing more hair to drop once you understand some facts about female hair loss. You see, in many cases, female hair loss is just a temporary occurance and hence, finding a hair loss solution to it is relatively easy.

Androgenetic alopecia is the hereditary form of baldness that affects 50 percent of men, and some women after 40. Female hair loss usually starts after menopause although it can begin earlier. The main reason for this is that estrogen levels decline. Hormonal changes cause hair to thin.

It is comforting to note that other than androgenetic alopecia, the most common cause of hair loss in women, is a result of metabolic and hormonal changes. Thus, the hair loss is usually temporary. Also, unlike men, women rarely become totally bald. However, what females generally experience is the thinning of their hair.

For example, in the case of pregnancy, hair loss is temporary and should stop about 6 months after birth. When a woman is pregnant, a large amount of oestrogen is produced causing the hair follicles to go into their growth phase.
Once the baby is born, the woman’s hormonal balance is restored. The reverse now happens with the hair follicles going into a hair loss phase.

While nothing much can be done to prevent hair loss during this period, applying hair tonic to hasten hair re-growth can be a helpful solution.

Crash diets leading to rapid weight loss over a short period of time prompts excessive hair loss.

Physical and emotional stress can cause hair loss but this usually only occurs after a prolonged period of time and in extreme cases. Once stress levels are restored to normal levels, hair loss should stop. Thus, a good hair loss solution is to find ways to reduce stress!

Certain drugs can also cause hair to shed. The most common medical treatment that causes hair loss is chemotherapy. The drug medication attacks the hair cells, causing hair loss from the scalp. Certain prescription drugs (for thyroid hormone deficiency, diabetes and lupus) and dieting supplements are also causes of hair loss.
Once these drug medications are stopped, the hair loss problem should disappear.

Other stresses to the hair may include frequent dyeing and chemicals eg. perming solutions applied to the hair. Generally, healthy hair can undergo these treatments without showing signs of stress, if they are not done too often. But if hair is not allowed a chance to recover from the constant application of hair chemicals, then it becomes brittle and starts to break off. Hair loss treatment products to help strengthen the follicles should help when applied to the scalp.

Once you understand what is happening to your hair, then seek out the right hair loss solution. In most cases, if the cause is temporary, then you can be assured that your problem will pass with the right remedy actions. However, for the more serious cases, do seek the professional help of a licensed dermatologist for a complete diagnosis and treatment plan.

WHAT IS LASER HAIR REJUVENATION THERAPY?
Laser Hair Rejuvenation is a “NEW” non-surgical scientific approach to treating hair loss, thinning hair, baldness and problems associated with the scalp. Laser Hair Rejuvenation is a tested method using low level laser energy, or “soft” laser light to effectively treat the appearance of hair loss.

The Laser Hair Rejuvenation non-surgical treatments utilize a device containing therapeutic low level lasers developed in Europe. The treatment delivers light energy directly from rotating laser positions designed to cosmetically treat the appearance of thin, fine, damaged hair. Physicians are praising this technology as an effective treatment.

THE EVOLUTION OF LASER HAIR REJUVENATION
In 1964, Professor Andre Mester began experimenting with the use of low-power laser energy in Budapest, Hungary. In his experiments with wound treatment on mice, he noticed rapid healing due to microcirculation of blood supply. This healing was also obvious in Laser Hair Rejuvenation treatment of diabetic patients suffering with dystrophic sores. He was amazed to find sores that would not otherwise heal were healed, and he also observed the appearance of accelerated hair growth and thickening of hair in the treated areas. Today NASA is using Low Level Lasers to heal wounds of there astronauts in space.

This theory through its evolution has since been refined and is widely becoming one of the most popular non-invasive hair loss treatments.

WHAT CAUSES HAIR LOSS OR THINNING HAIR?

· Heredity

· Male Pattern Baldness

· Diet

· Medications

· Postpartum Changes

· Menopausal Changes

· Excessive Chemical Processing

· Stress

WHAT ARE THE BENEFITS OF LASER HAIR REJUVENATION?

· Maintains the appearance of the hair in 85%-93%

· Achieves fuller, thicker, healthier looking hair.

· Effective in both men and women.

· Success in all ages.

· No Discomfort.

· No Negative side effects

· No medications

· Increases the appearance of the hairs volume and strength

WHO IS THE BEST CANDIDATE FOR LASER HAIR REJUVENATION?

Generally men in the early stages of male pattern baldness and women who are experiencing a general thinning of the density throughout. Laser Hair Rejuvenation is the long anticipated answer for the hair transplant specialist challenged by the ineligibility to perform transplant procedures on certain individuals primarily due to the following scenarios:

a) Patients with early stages of hair loss. (Thinning areas where the existing hair is still too close together to warrant an incision between the existing hairs.)

b) Women that experience general thinning throughout including the sides and back area where a transplant specialist would typically harvest the donor site.

IS LASER HAIR REJUVENATION SAFE?

Yes! The Laser Hair Rejuvenation devise, (which is classified as a Certified Class IIIA cosmetic laser) is recognized by the FDA and is approved for cosmetic use in the United States.

The energy produced by the photons does not have the thermal component to cause thermal injuries to users or operators.

HAVE THE BENEFITS OF LASER HAIR REJUVENATION
BEEN SCIENTIFICALLY MEASURED?

Yes! A double-blind placebo controlled study was conducted to evaluate the effect of Laser Hair Rejuvenation by comparing it with a placebo laser. All patients with the exception of one in the laser treated group showed a complete stop of hair loss. All patients except three showed a clear re-growth of hair with a reduction of at least one category in the Norwood Classification. Out of 18 patients, 14 showed an increase in hair thickness and all 18 showed improvement in the general hair shaft quality. The results showed no improvement in the placebo group or any adverse effects of the treatment. The present double-blind study confirmed by histological examination, revealed that re-growth of new hairs can be achieved in the most middle-age and younger males with typical male pattern baldness, when the scalp is irradiated with Laser Hair Rejuvenation for 30 minutes twice a week for 5 weeks.

There are many different types’ hair extensions available today, such as weaves, clip in, adhesive based fusion and metal tubing.

Clip in hair extensions are the least damaging and easiest to apply. Clip in hair extensions can give you more length, volume, texture and also allow you to add colour. You clip them into your own hair by separating your hair and starting at the nape of the neck. The amount of clips you use is up to you depending on the level of volume that you require. You simply clip the extensions to your own hair, working your way up your head. You open the clips and face them to the scalp then snap them into place.

For best results with clip in hair extensions you should style your own hair and the hair extensions separately. Clip in hair extensions can be worn all day and night, however you should take them out before sleeping. Clip in hair extensions are sometimes just worn for a night out and other people wear them everyday. This is because clip in hair extensions are versatile and quick to put in and take out. You don’t require a professional to fit them for you; you can do it yourself at home. There is no messing around with glue, meaning there is no damage to your hair. Clip in hair extensions are also reasonable in price. They are one of the most affordable forms of hair extensions but do not lack in quality because of this, most are made using 100% human hair. You can choose from lengths such as 11" through to 28".

Another form of hair extensions is bonding/sealing. This is when your real hair is sectioned and the extensions are plaited to your own hair and painted with a bonding solution, this seals the extension around the hair. This form of hair extension can last up to four months, after the four months the bond starts to loosen. This type of extension must be attached by a professional as they can be extremely difficult to do at home yourself. If done yourself at home the attachments can be hard to handle and you run the risk of ruining your hair as well as getting uneven length that will look un-professional.

One other method that you may or may not be aware of is when it comes to hair extensions is the fusion method. This is when 20-50 strands of human hair extensions have warm protein glue placed on them and a plastic template separates sections of the hair, keeping it away from the scalp. Each piece of hair extension is attached with glue to your natural hair and the attachment of hair to hair extension is pulled through a hole in the plastic template. You should roll the extensions and you’re natural hair together until a smooth bond is formed. This type of hair extension is a very long process and has to, with no exceptions be done on clean, washed hair, without the use of conditioner, otherwise the extensions won’t stick correctly. When the time comes to remove hair extensions that have been fused on you should always get it done at a salon as the bond between the hair and extension needs to be softened and cracked; this is done with fusion remover and specialist tools

The last form of extensions that I’m going to mention is Bonding Weaves. These are very similar to clip in extensions; you cut a length of human hair to fit your head shape. The extensions are then glued to your head just below the parting. You can get the glue to do this from most hair and wig stores. This form of hair extension is safe to do at home and the removal of these hair extensions can also be done at home as the remover can also be purchased from the same stores as the glue. You should make sure you have the remover before you put the extensions in so you can guarantee you can get them out when needed.

There are many other forms of hair extensions, but the ones mentioned above are the most common and the ones you are most likely to come across when searching for the right type for you.

Emo Hair

By: Burt Cotton

Emo, or the emotional punk movement, includes its own attitude, fashion and values along with emo hairstyles. The emos refuse to conform to the mainstream and adhere to the dictates of current fashion. As a result, emo has created its own fashion, and emo haircuts are distinctive with jet black hair, asymmetrical styles and bold highlights. Being emo means standing out from the crowd, and emo haircuts are often experiments created by friends rather than the kind of "dos" that can be created at a salon. You might even want to create your own emo hairstyles with a pair of scissors, dye and a razor.

With emo hairstyles, anything goes, as long as it doesn't look like something the prom queen or king would wear. Almost all emo women dye their hair jet black. Many emo men have jet black hair, but it is not universal. Any black dye will do, and the bolder the effect, the better. You need not be concerned about the dye job looking natural, since many emo hairstyles are obviously dyed. You might want to touch up the edges with frosted highlights, purple or blue, but make sure these colors show through the black dye.

Emo hairstyles do not require a professional stylist, and you are better off using your little brother's kindergarten scissors or a pair of shears than scissors designed for cutting hair. Don't worry about the hair being uneven; the more asymmetrical, the better. Many emo hairstyles feature a patch of triangular hair around the face or lopsided hair. It is a wonder how some emo hairstyles can allow their wearers to see anything, but most emos do manage to cross the street safely.

You may try shaving all of your hair in back and leaving hair only on the top or on the sides. Some may also decide to shear their hair on the sides as well. For these emo hairstyles, use electric shears used for giving army "buzz" cuts. Although much of the hair will be buzzed off, there should still be a flamboyant black tousel hanging down the front into the eyes.

Emo hairstyles are definitely shaggy, and sometimes spiky, so use scultping gel to your advantage. It is a good idea to find the most potent gel on the market, especially if you want to create spikes. Simply make the hair spiky or shaggy when wet by working a gel through your hair and teasing out the bold effects. Dry your hair, adding more gel and spray until your spikes are absolutely stiff. Do not brush. Emo hairstyles are not for the faint of heat, so don't worry about having a stiff head of hair after all of that spray. In any case, it can be shampooed out.

To create emo haircuts, you do not need a lot of expensive hair equipment like curling irons or flat irons, but you will want to experiment with different gels and sprays. The best kind of gels make the hair actually stiff and spikey. If you are not satisfied with your haircut, you can always trim yourself using an ordinary pair of scissors. Although it is sometimes better to have someone else style your hair, because we cannot always get a good perspective on how our hair looks from the back. Don' t worry if the cut is not even or straight, since emo hairstyles tend to be asymmetrical.

If your hair is long or medium length, create an uneven look by cutting one side shorter than the other, or putting part of your hair up and leaving the other half down. Use a special spray for super shine and add highlights if you have layers. Although shorter emo hairstyles are more common than long, you can still look one hundred percent emo and have long locks.

The way photocopiers have changed in the last ten years is impressive, but this move into multifunctional digital devices has caused some problems with accessibility. The large operating buttons have been replaced by small touch screens and many people have found the lids much heavier to lift. New features have been developed recently which have the potential to drastically improve accessibility for disabled users.

Photocopiers have changed drastically in the last ten years, now being able to perform many more tasks than just duplicating hard copies of documents. These improvements in technology mean that many photocopiers use a built in hard drive so function in a different way to some older photocopiers. Although impressive, these changes sometimes cause a few problems with accessibility for disabled users, especially as small touch screens have replaced the old large, easily recognisable buttons. Photocopiers can perform many more functions and so need a screen to display all the options but this change meant that some people found them hard to access. However, a few photocopier manufacturers have now come up with ways to improve the accessibility of photocopiers.

Voice guidance kits are now available for visually impaired users which often work by having a small speaker attached to the side of photocopiers and use speech-output software to communicate with a user. The number pad is used to go through various functions, with the speech output guiding the user. Braille label kits can also be provided for some devices, making tactile buttons recognisable.

Automatic document feeders that are now included in photocopiers increased the speed at which multiple documents can be copied. However, where there used to be just a straight forward platen cover covering the platen glass, these often now incorporate automatic document feeding equipment, making them heavier to lift and especially difficult for wheelchair users. To combat this problem, an accessibility handle can be fitted to many photocopiers which makes lifting the document feeder much easier.

Touch screens can also be a problem as they are designed to be viewed from above and sometimes do not have very high contrasts making them difficult to access by wheelchair users and people with low vision. This problem has again been addressed by some photocopier manufacturers. The majority of photocopiers can now be networked. Software such as Remote Operator's software kit can be installed on the photocopiers, allowing users to access from a laptop or desktop PC, where the contrast and size of the information can be changed to make it more easily readable.

A number of measures have been taken to improve accessibility of photocopiers. Technology in photocopiers and their capabilities has improved vastly in the last ten years and should be accessible to everyone. Now that a few photocopier manufacturers have taken measures to improve accessibility for disabled users it should mean that more people will be able to take advantage of these new features and access them easily.

For more information about the accessability of photocopiers please click here.

Hayden Panettiere has revealed that media comparisons to Lindsay Lohan have made her adamant not to follow in her fellow starlet’s partying footsteps.

Heroes star’s beauty, blonde locks and small stature

as well as her constantly photographed life in Hollywood - have resulted in inevitable comparison to La Lohan but Hayden insists she won’t be falling out of nightclubs any time soon.

She told Teen magazine recently:

“For God knows what reason, they compare me with Lindsay Lohan [and so] it’s kind of become, ‘All right, you guys can stay there and try knocking me off my horse.’

alopecia-1.jpg (27446 bytes)Alopecia means hair loss.

Alopecia areata is a particular disease affecting hair growth in the scalp and elsewhere. This loss is the cause of great concern to those affected, since the hair is the most important part of our "body image".

WHO GETS ALOPECIA AREATA?: The condition can commence in children, and is most common in teenagers and young adults. It can occur for the first time in old age. Most studies show that males and females are equally affected.

IS IT INFECTIOUS?: No. There is no way that a person with alopecia can transmit the problem to anyone else, and it is not "caught" by using someone else's comb or towel.

IS ALOPECIA AREATA INHERITED?: This condition usually affects one member of a family. As will be discussed later, some related conditions can appear in other family members, and occurrence of alopecia in relatives would be possible but unlikely.

IS IT COMMON?: No figures are available to answer this question - but 1-2% of new patients seen by dermatologists have alopecia areata. It is perhaps best to say it is not "uncommon".

HOW DOES IT START?: Usually a bare scalp patch is noticed by the patient - or by hairdressers or relatives. It is unusual for the lost hair to be seen on the comb or on washing the hair.

WHAT AREAS ARE AFFECTED?: The scalp is the usual area, but the beard in men, and eyelashes or eyebrows, may be affected alone or together with scalp hair loss. In the uncommon severe forms, body hair may be lost as well. The only other structures affected are the nails - and people with severe alopecia areata may show dull and rigid finger nails.

WHAT DOES IT LOOK LIKE?: The patches are smooth, with few remaining hairs in the centre. Round the edge of the patch stub-like hairs can usually be seen (often called "exclamation mark" hairs as they are they are thicker at the tip than at scalp level).

CAN THE LOSS BE DIFFUSE?: Less commonly, some hair may be lost over wide areas, causing general thinning of the hair. This can cause the hair to "turn white overnight" by selectively affecting dark hairs and leaving grey hairs.

WHAT CONDITIONS CAN LOOK SIMILAR?: In children, tinea (ringworm) can produce hairless patches, but usually the hairs are broken and not lost, and the area is inflamed. The condition which most closely resembles alopecia areata occurs with the habit of "hair pulling" when this is confined to one area and is severe. Scalp diseases with scarring or diffuse hair loss due to other causes are significantly different from alopecia areata.

HOW IS IT DIAGNOSED?: The diagnosis is made from the story of loss and seeing the typical patch of bare skin. It is very rare for tests to be needed to make a diagnosis.

WHAT TESTS CAN BE PERFORMED?: Hair can be plucked and examined under a microscope, and if tinea is suspected, hairs may be taken for culture. For very careful study, a small biopsy may be performed for microscopical examination of the scalp skin. In most cases, tests are performed only if there is a need to access general health: blood tests do not show any abnormality in alopecia areata.

WHAT CAUSES ALOPECIA AREATA?: The mechanism is known- inflammation develops round hair roots, and the hair roots become inactive and shrink in size. However the cause of this inflammation is not understood.
It is safe to say that no simple explanation such as deficiency of vitamins or other food, methods of hair care or external injury can be given. The cause is not any fungal, bacterial or viral infection, and it cannot be transmitted to any other person. It is likely that eventually the condition will be found to be "auto-immune", with another body tissue attacking the hair roots.

DO NERVES CAUSE ALOPECIA?: A severe shock can definitely set off an attack, but there is much less certainty that ordinary stresses of life affect the condition. Of course, the person often becomes more tense and nervous through worrying about the problem itself.

WHAT CONDITIONS CAN ACCOMPANY ALOPECIA?: In children, alopecia areata may occur more often, or more severely if the child has eczema. In adults it is known that the patient or the relatives are slightly more likely to show other auto-immune conditions than is the general population. These include colour loss (vitiligo) and thyroid disorders.

DOES ALOPECIA AREATA AFFECT GENERAL HEALTH?: No. All aspects of general health are unaffected - apart from the rare associations with other diseases. It usually occurs in healthy young people.

WHAT HAPPENS AFTER ALOPECIA DEVELOPS?: It is thought that the majority of cases settle down and regrow after one or more patches have been present for some months.
However, sometimes the condition persists, and new patches can appear while old patches regrow. Sometimes the affected area extends as a ring around the regrowing area.
In the worst cases the condition progresses (gradually or quickly) until large areas of hair have been lost. These cases naturally bring most distress to the people affected.
When the condition first appears, it is not possible to predict whether it will be mild and recover soon or become severe.

IS IT KNOWN WHY SOME CASES BECOME SEVERE?: Apart from the uncommon type which is accompanied by eczema in children, no information is available about this. Everyone hopes that early treatment will stop the progress of the condition but if the alopecia is destined to be severe, this course may still be followed despite treatment.

TREATMENT

Many treatments have been used over the years and in different countries, and it is certain that new and improved treatments will be discovered. The success of D.N.C.B. treatment (to be discussed below) in some people, has awakened new research interest in alopecia areata and new study techniques are being employed.

The paragraphs that follow give some general information, but obviously full detail cannot be given here.

FOOD AND VITAMINS: There is no evidence that any deficiency exists, or that changing diets will be helpful.

ZINC TREATMENT: There have been medical articles from Central Europe stating that prolonged treatment with zinc sulphate is helpful. There is no zinc deficiency discoverable in the body, and short term treatment with zinc is not helpful.

HAIR CARE: There is no special shampoo to use or avoid and hair styles will be chosen by each person to cover the patch. Scalp tonics and hair conditioners do not help.

IRRITANT CHEMICALS : FREEZING : ULTRAVIOLET LIGHT: These methods have been used and in early mild cases may have some influence. The most recent chemical described in this group is "dithranol" which probably acts by irritating the scalp and not by specific effect.

CORTISONE CREAMS AND LOTIONS: These agent s are commonly used, sometimes with a plastic film applied over the scalp to assist penetration of the cortisone. This treatment is quite safe.

INJECTION OF CORTISONE INTO PATCHES: This is standard treatment, especially for early cases which affected small areas. It seems to give the best quick results - speeding recovery in mild cases, but is less effective in more severe cases. The total amount of cortisone used is not high and does not produce any adverse affects.

CORTISONE TABLETS: There is no doubt that taking cortisone tablets can produce regrowth in many people. However, the regrowth may only last as long as the cortisone tablets are continued - and long term cortisone treatment can affect many aspects of general health. Therefore, most doctors advise against cortisone tablet treatment except in severe and progressive alopecia in adults

D.N.C.B. TREATMENT: D.N.C.B. is a chemical (dinitrochioro benzene), which rapidly produces skin sensitivity. In some people with severe alopecia, continued application of D.N.C.B (enough to produce a continuing rash) can result in regrowth of hair.
This method is not advised for less severe cases, and it does not always work - but it does suit a number of people.

P.U.V.A TREATMENT: The success of using photosensitizing "psoralens" with ultraviolet light in the A range (P.U.V.A) in other skin conditions has suggested it could be used in alopecia areata. There have been some encouraging reports, but long term studies have yet to show what success this treatment will have, and who should be treated by this method.

TREATMENT OF NERVOUS PROBLEMS: This is an important part of treatment when people with alopecia are under great stress, or are upset by the problem. Doctors differ in their opinion about using sedatives or mild tranquillisers for alopecia sufferers who are not obviously affected by stress and problems.

REGAINE (TOPICAL MINOXIDIL): Minoxidil was first used in tablet form for the treatment of high blood pressure. Hair growth was observed as a side effect in patients taking the preparation for this condition. Tests were then carried out to see if monoxidil could be applied to the scalp, as a topical (liquid) preparation. Clinical trials involving thousands of men established the efficiency of Regaine® (topical minoxidil) in treating alopecia androgenetica (male pattern baldness) for a percentage of men. Regain® is currently indicated for the treatment of male pattern baldness. Those most likely to respond include men who are balding on the crown rather than temples, and who have been balding for less than ten years.

In addition, there have been reports of Regain®'s effectiveness for alopecia areata. Regaine® takes 4- 6 months to be effective and it is available by prescription in Australia at a cost of approximately $70 per month (Some of this is reimbursed by private health care funds.)

WIGS: for children and adults with severe alopecia, wearing a wig may become the best "treatment". Wigs do not stop hair from regrowing and do not make the problem worse.

In 1981 the government introduced a scheme for wigs to be obtained free of charge for people with severe alopecia and details may be obtained by your doctor. It is to be hoped that this valuable service will continue to be available for future years and under full medical funding arrangements.

Satellites currently use radio waves to exchange data. Now the data rate has been increased a hundredfold by using lasers instead of radio signals. Two test satellites each carried a diode laser pump module developed with the help of Fraunhofer researchers.

The data whizzed back and forth at the speed of light between German satellite TerraSAR-X and US satellite NFIRE, covering more than 5000 kilometers in space without any errors. What was special about this space test recently performed by Tesat-Spacecom was that the data was transmitted by laser.

The bandwidth achieved in the test was a hundred times greater than during conventional communication by radio waves, enabling a data rate equivalent to roughly 400 DVDs per hour. This could make it possible to transmit large data packets between several satellites in the future, for instance to send image data from Earth observation satellites to a ground station. That has not been possible until now, as the bandwidth of radio waves is not large enough.

Another advantage of this new form of communication is that lasers are easier to focus than radio waves, which means that data transmissions can be directed more accurately.

The communication lasers on board the satellite are actuated by pump modules, which were developed to a large extent by researchers at the Fraunhofer Institute for Laser Technology ILT in Aachen on behalf of Tesat GmbH & Co. KG as part of a program financed by the German Aerospace Center (DLR).

“The modules have to withstand the vibrations and forces of acceleration on board the satellites during the launch and must then survive the inhospitable conditions in space – such as extreme radiation and strong temperature differences,” says Martin Traub, who led the developments at the ILT.

“We therefore tested the pump modules under extreme conditions in advance, subjecting them to temperatures of -35°C to 60°C, acceleration forces 1300 times as strong as those of the Earth, and gamma rays.” The modules mustn’t be too big or too heavy for use in space: Measuring 5 x 5 x 2 centimeters, they are barely larger than a matchbox, and weigh little more than a bar of chocolate at 130 grams.

“We achieved this minimal weight by selecting the right materials and a sophisticated housing: Any material that wasn’t absolutely essential was milled away,” says Traub. The major challenge is that, despite the reduced weight, the heat generated by the laser’s several-watt output still has to be dissipated.

Lichen Planus

Lichen Planus

Lichen planus is an uncommon skin complaint. It is thought to be due to an abnormal immune reaction provoked by a viral infection (such as hepatitis C) or a drug. Inflammatory cells seem to mistake the skin cells as foreign and attack them.

Lichen planus may cause a small number of skin lesions or less often affect a wide area of the skin and mucous membranes. In 85% of cases it clears from skin surfaces within 18 months but it may persist longer especially when affecting the mouth or genitals.

Clinical features

Lichen planus takes several forms.

Classical lichen planus
Classical lichen planus is characterized by shiny, flat-topped, firm papules (bumps) varying from pin point size (‘guttate’) to larger than a centimetre. They are a purple colour and often are crossed by fine white lines (called ‘Wickham's striae’). They may be close together or widespread, or grouped in lines (linear lichen planus) or rings (annular lichen planus). Linear lichen planus can be the result of scratching or injuring the skin. Although sometimes there are no symptoms, it is often very itchy.

Lichen planus may affect any area, but is most often seen on the front of the wrists, lower back, and ankles. On the palms and soles the papules are firm and yellow. Very thick scaly patches are particularly itchy and are most likely to arise around the ankles (hypertrophic lichen planus).

New lesions may appear while others are clearing. As the lichen planus papules clear they are often replaced by areas of greyish-brown discolouration, especially in darker skinned people. This is called postinflammatory hyperpigmentation and can persist for months.

Oral lichen planus
The mouth is involved In 50% of cases and is often the only affected area. The usual areas affected are the inside of the cheeks and the sides of the tongue, but the gums and lips may also be involved. The most common features are:

  • Painless white streaks in a lacy or fern-like pattern
  • Painful and persistent ulcers (erosive lichen planus)
  • Diffuse redness and peeling of the gums (desquamative gingivitis)

In some cases oral lichen planus affecting the gums is due to contact allergy to mercury in amalgam fillings on nearby teeth. The cause can be confirmed by patch testing. In these patients the lichen planus may resolve on replacing the fillings with composite material. If the lichen planus is not due to mercury allergy removing amalgam fillings is very unlikely to result in cure.

Vulval lichen planus
As in the mouth, lichen planus may cause painless white streaks. Erosive lichen planus is more common and is one cause of vulvodynia (burning discomfort of the vulva). Erosive lichen planus affects the labia minora (inner lips) and introitus (entrance to the vagina). The affected mucosa is bright red and raw. The labia minora can shrink and stick to each other or to the labia majora (the outer lips). Erosive lichen planus can be very painful, preventing sexual intercourse. It can also scar, closing over the vagina.

Sometimes lichen planus affects deeper within the vagina where it causes desquamative vaginitis. The surface cells in the vagina peel off and cause a mucky discharge. The eroded vagina may bleed easily on contact.

Penile lichen planus
Classical papules are the most common form of lichen planus on the penis and mostly occur in a ring around the glans (the tip of the penis). White streaks and erosive lichen planus are much less common on the penis.

Other mucosal sites
Erosive lichen planus uncommonly affects the eyelids, external ear canal, oesophagus, larynx, bladder and anus.

Lichen planopilaris
Follicular lichen planus, also known as lichen planopilaris, results in tiny red spiny papules around a cluster of hairs. Rarely, blistering occurs in the lesions. Permanently bald patches may develop. Sometimes no follicular scaling or inflammation is present but bald areas of scarring slowly appear, often looking rather like footprints in the snow. This is known as ‘pseudopelade’.

Frontal fibrosing alopecia is thought to be a limited form of lichen planopilaris.

Lichen planus of the nails
Lichen planus affects one or more nails in 10% of cases, sometimes without involving the skin surface – if all nails are abnormal and nowhere else is affected it is called twenty nail dystrophy. The nail plate tends to thin and may become grooved and ridged. The nail may darken, thicken up or lift off the nail bed (onycholysis). Sometimes the cuticle is destroyed and forms a scar (pterygium). The nails may shed, stop growing altogether and rarely, completely disappear.

Lichen planus pigmentosa
In some patients oval greyish brown marks appear on the face and neck or trunk and limbs without an inflammatory phase.

Actinic lichen planus
Actinic lichen planus only affects sun exposed sites such as face, neck and the backs of the hands.

Bullous lichen planus
Bullous lichen planus is rare; blisters appear within lichen planus papules or by themselves, generally on the lower legs.

Lichenoid drug eruption

Lichenoid drug eruption refers to a lichen planus-like rash caused by medications. It tends to cause asymptomatic or itchy pink or purple flat slightly scaly patches on the trunk, but the oral mucosa and other sites are also sometimes affected. Many drugs can rarely cause lichenoid eruptions but the most common are:

Actinic lichenoid drug eruption is confined to sun exposed sites. The most likely drugs to cause this are quinine, taken for leg cramps, and thiazide diuretics, used for hypertension and heart failure.

Lichenoid drug eruptions clear up slowly when the responsible medication is withdrawn.

Skin cancer

Rarely, longstanding erosive lichen planus can result in skin cancer (squamous cell carcinoma) of the mouth (oral cancer), vulva (vulvar cancer) and penis (penile cancer). This shold be suspected if there is an enlarging lump or an ulcer with thickened edges.

Skin biopsy

The diagnosis of lichen planus is often made by a dermatologist, oral surgeon or dentist by the typical appearance. However, a biopsy is often recommended to confirm or make the diagnosis and to look for cancer. The histopathological signs are of a ‘lichenoid tissue reaction’ affecting the epidermis (the skin cell layer). Typical features include:

  • Irregularly thickened epidermis
  • Degenerative skin cells
  • Liquefaction degeneration of the basal layer of the epidermis
  • Band of inflammatory cells just beneath the epidermis
  • Melanin (pigment) beneath the epidermis

Direct staining by immunofluorescent techniques may reveal deposits of immunoglobulins at the base of the epidermis.

Treatment

Treatment is not always necessary.

Potent and ultrapotent topical steroids
Topical steroids such as clobetasol proprionate and betamethasone proprionate ointments are generally applied for 4 -6 week courses. A thin smear should be rubbed in accurately once a day and stopped when the lesions have flattened with the normal skin. Brown marks are often left at the sites, which take several months to fade.

In the mouth, steroid pastes or inhalent powders may be easier to apply to affected sites. Hydrocortisone foam can be used inside the vagina.

Steroid injections into affected areas may be useful for localised disease.

Systemic steroids
In extensive cases systemic steroids such as prednisone may be prescribed for a few weeks or longer. This will lessen the itch and often clear up the lichen planus completely. However, it may recur later. Systemic steroids may have serious side effects, so discuss this treatment with your dermatologist.

Other treatments include long term antibiotics, oral antifungal agents, phototherapy, acitretin, methotrexate and hydroxychloroquine. The immune modulating drugs that inhibit calcineurin, tacrolimus ointment and pimecrolimus cream, may be useful for oral and genital lichen planus.
